Third party consent
To action any request for a third party to act on your behalf, we require our specific authority form to be completed. This form can be found below and printed or you can contact us for a copy.
Once completed, this needs to be brought in by yourself, in person, with two forms of identification. One showing your address, such as a driving licence or utility bill/bank statement (less than 3 months old) and a photo ID, such as a passport or driving licence. We also require ID for the third party you wish to have access to your information.
We apologise for any inconvenience but this is necessary for GDPR and our duty of care protocol.
Online proxy access
What it is: Enables parents, family members, or carers to manage some NHS services for another person with their consent. This is usually using the NHS App or Patient Access app.
How to set up:
- Download our proxy access forms below and please discuss with the person who’s consent is required. Complete the forms with them and return to the surgery (preferably to the front desk with a form of ID).
- Once we have received the completed forms and have confirmed that the patient has given their consent, we can authorise the connection to their online health record for the named person.
- You can start to use the linked profile in your NHS App or Patient Access App to manage their online services.
Please be aware that currently the NHS App only allows proxy access to patients registered at the same surgery. If you are registered at a different surgery to the person you are assisting then you will need to use the Patient Access App. For further guidance on this please follow the link below.
